IUI or IVF: Which Comes First in Hyderabad?
IUI places prepared sperm into the uterus around ovulation. IVF retrieves eggs, fertilises them in the laboratory, and transfers an embryo. IUI is often considered when the fallopian tubes are open and semen parameters are suitable. IVF is considered when tubes are blocked, sperm parameters are severely reduced, simpler treatment has not worked, or eggs need to be fertilised outside the body. There is no fixed rule that every couple must try IUI first.
What IUI involves
Intrauterine insemination (IUI) prepares a semen sample and places it in the uterus close to the time of ovulation. It may be paired with medicines that help an egg mature. IUI does not retrieve eggs and does not create embryos in a lab. It still needs at least one open fallopian tube, because fertilisation happens inside the body.
What IVF and ICSI involve
IVF uses hormone stimulation, egg retrieval, fertilisation in the laboratory, and embryo transfer. ICSI is a form of IVF in which one sperm is injected into each mature egg. ICSI is commonly used when the sperm count or motility is very low, or when previous fertilisation in the lab was poor. Embryo transfer can be in the same cycle or later, after freezing, depending on the lining, hormone levels, and the doctor’s plan.
| Question | IUI | IVF |
|---|---|---|
| Where fertilisation happens | Inside the body | In the laboratory |
| Open fallopian tube needed | Yes | No |
| Egg retrieval | No | Yes |
| Typical place in a plan | Often an earlier, simpler step | When IUI is unsuitable or has not worked |
Situations that often point toward IUI
- Unexplained infertility with open tubes and a suitable semen report.
- Mild male-factor findings, after the full report is reviewed.
- Ovulation treatment is already planned and insemination may add a benefit.
- Donor sperm is being used and tubes are open.
Age still changes this choice. A woman in her late 30s or 40s may be advised to move to IVF sooner, because repeating several IUI cycles uses months that matter.
Situations that often point toward IVF
- Both fallopian tubes are blocked or absent.
- Semen parameters are severely low, including some cases of azoospermia after urology review.
- Several well-timed IUI cycles have not led to pregnancy.
- There is a plan for embryo genetic testing.
- Previous IVF needs a different protocol. That is a separate consultation, not an automatic repeat of the old plan. See recurrent IVF failure.
Questions that decide the order
Before choosing, the clinic usually needs a semen analysis, an ultrasound, and enough information about the tubes. A hysterosalpingogram or a hysteroscopy is requested only when the history calls for it. Ask which missing test would change the choice between IUI and IVF. That single question prevents starting the more intensive treatment before the simpler one has been fairly considered, and the reverse.
Common questions
Is IUI always tried before IVF in Hyderabad?
No. IUI is skipped when tubes are blocked, sperm parameters are severely low, or age and history make a simpler cycle a poor use of time.
Can IUI work if the fallopian tubes are blocked?
No. IUI needs a path from the uterus to the ovary. Blocked tubes are a reason to discuss IVF.
What is the difference between IVF and ICSI?
Both retrieve eggs and create embryos in the lab. ICSI injects one sperm into each mature egg and is used mainly for significant male-factor infertility or poor fertilisation.
